Prior to scheduling an appointment with our office, we must first determine whether you are a candidate for possible surgery with Dr. Terzis.

First, we will need a detailed description of the patient’s injury and/or diagnosis and the date it occurred. Results of all prior testing and surgeries are also needed to assist us in making our determination.

We request that all previous medical records be forwarded to our office, to include:

• Operative reports
• Pathology reports
• CT and MRI films and reports
• X-rays, scanograms
• Orthopedic reports
• Rehabilitation reports

Once Dr. Terzis and her team have reviewed the records and have determined that a patient is a candidate for possible surgery, the Patient Coordinator will contact the patient to schedule an office consultation with Dr. Terzis.
