• Video and Photo Protocols
Patients should send a video (using the examples below as a guide) to document their movement and abilities. Send to Julia K. Terzis, M.D., Ltd., 330 W. Brambleton Ave. Suite One, Norfolk, VA, 23510 USA.

It is preferred that the video be shot on a Mini-DV cassette tape. Do not send a VHS tape as the quality is not fit for the doctor to evaluate.

For Photos / Video: Note the following instructions on how to frame your face vertically in the viewfinder. Remember to always make sure your face is clear and in focus.

Upper Extremity Paralysis Photo/Videos
Show the movement in your arm(s). Do not hold your paralyzed arm with your good arm. These movements will demonstrate your current range of motion with each arm. Face the camera and show your paralysis with your upper torso in the entire frame of your viewfinder.

Show arm(s) straight up above head
Show arm(s) straight out from your side (not in front of you but to your sides)
Show arm(s) touch in your mid chest with your elbow stick out to the side
Show arm(s) flexing up as if you are lifting something up in front of you
Show arm(s) touching the back of your head
Show arm(s) touching your mid back
Show arm(s) like you are showing me how strong you are

Show each of the movements again with your back turned to the camera.

Lower Extremity Paralysis Photo/Videos
Show yourself walking, first normally then secondly on your toes and heels. Do not worry about whether you can or cannot do anything with your paralyzed extremity. It is important to show your current range of mobility.
 
Sit in a chair and put your feet up on another chair and point your feet down and pull them up toward your as much as you can. Do this from both sides. Make sure you fill your camera viewfinder focuses on the feet. I need you to walk so that we can see both sides of your walk.

Facial Paralysis Photo/Videos
Close eyes softly
Close eyes hard
Raise eyebrows
Pucker lips as a kiss
Blow as a whistle
Smile broadly showing teeth
Smile slight showing no teeth
Stick you tongue out
Using your lower face muscles, show your bottom teeth

Put your face in the full frame of the camera (not from a distance, as we cannot see the detail in the face).
Smile broadly from the side of your paralysis and pucker your lips, close your eyes.
If possible, zoom in on the eyes and mouth to add a close up of eyes and smile.

Please try to have enough light and stay in focus. It will not help if your camera is out of focus as Dr Terzis uses this documentation to determine your needs.
